What is a CT brain plain scan?

It's a CT scan of the head done without contrast dye. You'll also see it written as NCCT brain, which means non contrast CT.

The scanner takes X-ray pictures from many angles and turns them into thin slices of your brain and skull. Your doctor can scroll through them like pages of a book.

What it shows well, and what it can miss

A plain CT brain is very good at some things and weaker at others. It helps to know both.

Shows clearlyCan miss or show poorly
Bleeding inside the skullVery early stroke changes in the first hours
Skull fracturesSmall changes in brain tissue
Swelling and fluid build upProblems at the back of the brain and brainstem
Large strokes after the first dayMany causes of long standing headache or dizziness
Shift of the brain from pressureNerve problems and small tumours

So a normal plain CT is reassuring after an injury. But it doesn't rule out everything. If symptoms continue, your doctor may ask for an MRI or a contrast CT.

Why doctors ask for it

Doctors commonly ask for a CT brain plain scan after:

  • A head injury, a fall or a road accident.
  • A fall in an older person, or anyone taking blood thinners.
  • A sudden, severe headache unlike any before.
  • A first seizure.
  • New confusion, drowsiness or a change in behaviour.
  • Follow up of a known bleed, or after brain surgery.

If someone shows stroke signs right now, such as a drooping face, a weak arm or slurred speech, go to a hospital emergency with a stroke team. Don't come to a scan clinic first. Treatment works best in the first few hours.

Plain or contrast CT brain?

Most head scans start plain. Blood shows up clearly without dye, and skipping the injection keeps the scan quick and simple.

Contrast is added when your doctor wants to look for infection, a tumour or problems with blood vessels. That's a different scan with different preparation. Check your prescription. If it says "plain", "NCCT" or "without contrast", this is the right page.

How to prepare

  • No fasting. Eat and drink as usual.
  • Keep taking your normal medicines.
  • Remove hairpins, earrings, spectacles and removable dentures before the scan.
  • Bring your prescription and any old head scans. Comparing them helps the radiologist.
  • If you are or might be pregnant, tell us before the scan.

What happens in the scan room

You lie on your back and your head rests in a padded cradle. The table slides into the scanner. It's a wide ring, not a tunnel, so most people don't feel closed in.

We ask you to keep your head still. The scanning takes seconds and makes a soft whirring sound. You're in and out of the room in about 10 minutes, and you can go home straight after.

Bringing in a parent or a child

Older people who fall and hit their head often seem fine at first. But bleeding can show up hours later, especially in people on blood thinners. That's why doctors often ask for this scan even when there's no visible injury. Bring their medicine list with you.

Children need to stay still for a few seconds. A parent can usually stay close, and we only scan children on a doctor's advice.

Is it safe?

A CT brain uses a small, controlled dose of X-rays. For a scan that can find bleeding quickly, it's a dose most doctors are comfortable with. Still, we scan only on a doctor's advice, and we keep the dose as low as the scan allows.

Your report

An MD radiologist reviews your images. The report usually reaches you within 24 to 48 hours on WhatsApp or email, and you can collect a printed copy too. Take it back to your doctor, who will explain what it means for you.
